Client Consent Form

Age. I confirm that I am 18 years of age or older.

Studio policies. I have read and accept the Inktention Tattoo policies. I understand that a tattoo is permanent, that I must disclose any medical condition, allergy, medication or pregnancy before the session, and that I must follow the aftercare instructions given by my artist. I understand that results may vary depending on my skin and my care.

Photos and video. I consent to the studio taking photographs and videos of my tattoo, the process and the final result, and I authorize their use in the studio's portfolio and social media.
